치과 올 세라믹의 발전과 그에 따른 특성의 변화 ②

서론
치과분야에서 메탈이 없는 올 세라믹 시스템은 임상가들의 요구에 맞추어 그 성능이 개선되어 왔다. 그리고 올 세라믹 보철물의 제작을 위해 열가압(heat pressing), slip cast, CAD/CAM과 같은 새로운 공정들도 더불어 발전되어 왔다.
특히 열 가압 성형을 이용한 보철물의 제작은 상대적으로 장비의 가격이 저렴할 뿐만 아니라 lost wax-up 기술이 응용될 수 있어 치과 기공사들이 선호하는 방법 중 하나로 알려져 왔다.
하지만 이러한 인기에도 불구하고 이 기술을 이용한 올 세라믹 시스템의 구체적이고 과학적인 정보들은 치과기공사들에게 매우 제한적이었다.
따라서 본 리뷰에서는 치과 올 세라믹 시스템 중 열 가압기술을 통해 제작되는 세라믹에 대해 전반적인 이해와 그 특성에 대해 기술함으로써 앞선 리뷰와 같이 치과기공사들에게 임상학술적으로 핵심적이라고 할 수 있는 정보를 제공하고자 한다.


본론
1991년 처음 열 가압 성형하는 세라믹이 소개된 이래로 이것은 심미적으로 쓰이는 단일 보철물 형태(라미네이트, 인레이, 온레이)로 7년 이상 임상에서 성공적으로 사용되어져 왔다[1].
초기의 1세대 열 가압 세라믹은 35~45 vol%의 leucite를 crystalline phase로서 포함한다[2](그림 1).
그리고 앞선 리뷰에서 언급한 feldspathic porcelain보다 두 배나 더 높은 굴곡강도와 파괴인성을 보였다[3].
이러한 강성과 인성의 증가는 열 가압과정 중에 오는 fine leucite crystal의 분산(dispersion)에 의해 설명된다[4].
추가로, fine leucite crystal과 glassy matrix의 열팽창계수의 차이로 인하여 냉각 시 compressive stress가 crystal주위에 발생하게 되고 이 stress들은 crack의 굴절(deflection)이나 기계적 성능의 증가에 기여한다고 알려져 왔다[5].

이 재료가 가지는 여러 기계적 특성의 장점에도 불구하고 고정성국소의치(bridge)보철물을 제작하는 것에는 그 강도가 충분하지 못하였다.
따라서 이러한 제한점을 극복하기 위하여 기존의 1세대 열 가압 세라믹과 같은 심미적 특성을 유지하면서 단일보철물 뿐만 아니라 고정성 국소의치(bridge)형태로도 제작이 가능한 2세대 열 가압 세라믹이 소개되었다(그림 2)(표 1).

2세대 열 가압 세라믹은 주요 crystalline phase로 65%의 lithium disilicate crystalline을 가진다. lithium disilicate 세라믹은 널리 연구되어 왔고[7-12] 많은 연구들이 lithium disilicate의 결정화가 이루어지는 메커니즘이 다소 복잡하다는 것에 동의한다[7]. XRD를 통한 결정상 연구들은 lithium disilicate 결정의 성장 전에 lithium metasilicate와 cristobalite의 존재를 확인하였고 lithium disilicate 결정들이 5㎛의 길이에 0.8㎛의 직경을 가지고 서로 엇갈려(interlock) 있다는 것을 보여 주었다(그림 3). 이렇게 층층이 겹쳐진 결정으로 엇갈려있는(interlocking) 미세구조는 2세대 열 가압 세라믹의 기계적 특성을 강화하는 것에 기여하는데, 이것은 crack의 전파가 분열되어 있는 면들(cleavage planes)를 따라가기는 쉬우나 crystal을 가로질러 가는 것이 어렵기 때문에 일어나는 복합적인 crack 굴절(deflection)로 설명된다.

몇몇 저자들은 열 가압 후에 lithium disilicate ceramic의 배열을 보고하였다[4, 13-14]. 그리고 이것의 결과로 높은 종횡비를 가지는 crystal로 인한 배열에 수직방향의 crack 전파을 가지는 기계적 특성을 언급하였다.
또한 Borom 등은 lithium disilicate crystal과 glassy matrix 사이에 열팽창 차이가 crystal 주위에 tangential compressive stress들을 만들어 crack의 굴절(deflection)이나 강도가 상승하는 것을 보고하였다[7].
초기에 lithia 기반의 ceramic은 조절이 불가능한 미세균열과 화학적 저항이 취약하고 부적절한 투명도를 가지는 단점을 보여주었다[15-18].
하지만 현재 lithium disilicate ceramic으로 만들어진 보철물은 높은 굴곡강도와 탁월한 심미성, 양호한 마모도등의 장점을 지니는 것으로 보고되었다. 또한 1세대 열 가압 세라믹이 보다 두 배 이상의 강도를 가지는 것으로 조사되었다.
전반적으로 all ceramic 시스템에서 lithium disilicate ceramic은 좋은 성능을 가지며 이러한 성능으로 인하여 기계가공으로 제조되는 보철물에서도 사용되어지고 있다(그림 4)(표 1).

결론
35~45 vol%의 leucite를 crystalline phase로서 포함하는 1세대 열 가압 세라믹은 feldspathic porcelain보다 두 배나 더 높은 굴곡강도와 파괴인성을 보인다.
하지만 이 재료가 가지는 여러 기계적 특성의 장점에도 불구하고 고정성국소의치(bridge)보철물을 제작하는 것에는 그 강도가 충분하지 못하였다.
따라서 단일보철물 뿐만 아니라 고정성 국소의치(bridge)형태로도 제작이 가능한 2세대 열 가압 세라믹이 소개되었다. 2세대 열 가압 세라믹은 주요 crystalline phase로 65%의 lithium disilicate crystalline을 가지며 층층이 겹쳐진 결정으로 엇갈려있는(interlocking) 미세구조로 인해 crack의 굴절(deflection)이나 강도가 상승하는 것을 보고하였다. lithium disilicate ceramic의 전반적인 좋은 성능으로 인하여 현재는 기계가공으로 제작되는 보철물에도 이용되어진다.
